Delaware First Health Grant 2026 – Apply Now

Delaware First Health and the Centene Foundation have opened submissions for their 2026 grant program. The grants aim to support Delaware-based nonprofit organizations and providers that address key drivers of health, such as housing insecurity, transportation access, and access to healthy food. By tackling these social and environmental barriers, the program seeks to improve the health and well-being of Delaware residents.

About the Delaware First Health Grant Program

Grants will provide funding to organizations working to reduce the impact of societal, environmental, and economic barriers on health outcomes. While all proposals addressing drivers of health are welcome, projects focused on housing instability or transportation accessibility will receive stronger consideration.

Funding Size

  • Total funding of $500,000 over the next three years
  • Minimum grant amount: $10,000

Who Can Apply

  • Delaware-based 501(c)(3) organizations
  • Must serve the Medicaid population
  • Must demonstrate how grant funds will be used and show outcomes over multiple points in time

Geographic Eligibility

  • Organizations must be based in Delaware

Application Process

  • Applications are due May 25, 2026
  • Award announcements will be made in September 2026
