About the Delaware First Health Grant Program
Grants will provide funding to organizations working to reduce the impact of societal, environmental, and economic barriers on health outcomes. While all proposals addressing drivers of health are welcome, projects focused on housing instability or transportation accessibility will receive stronger consideration.
Funding Size
- Total funding of $500,000 over the next three years
- Minimum grant amount: $10,000
Who Can Apply
- Delaware-based 501(c)(3) organizations
- Must serve the Medicaid population
- Must demonstrate how grant funds will be used and show outcomes over multiple points in time

- Organizations must be based in Delaware

Application Process
- Applications are due May 25, 2026
- Award announcements will be made in September 2026
